At Sojourn Hospice & Palliative Care we hold dear the momentous significance of our role in caring for the individual with a life-limiting illness. Our hospice care team includes:

Primary Care Physician

Your Primary Care Physician will work with you and your family to determine when it is time for hospice care.

Social Work

Our Clinical Social Workers often help patients complete advance directives, offer education about caregiving, and connect patients to community resources.

Hospice Medical Director

Our Hospice Medical Directors (Physicians) work with our team to oversee your care and assist with managing your pain and other medical symptoms.

Registered & Licensed Nurses

Registered and Licensed Vocational Nurses will be your direct care clinicians who will visit you and make sure you are comfortable and without pain. They will also work to reduce any secondary medical symptoms.

Hospice Aide

A Hospice Aide is available to assist you with basic self-care tasks including dressing, bathing and personal hygiene.

Chaplain, Minister or Spiritual Advisors

Chaplain, Minister or Spiritual Advisors provide comfort and counseling for you and your family, in addition to, coordinating with clergy of your own faith for support.

Physical, Occupational & Speech Therapists​

Physical, Occupational, and Speech Therapists are available to assist you with safety and managing your environment to make it more comfortable for you.

Volunteer Coordinators & Volunteers​

Our Volunteer Coordinator and Volunteers assist patients and families with daily tasks and activities to reduce your burden and save you time. They can also help with appointments, shopping or just listening and spending time with you.

Bereavement Counselors

Bereavement Counselors are there for the families of the patient to ensure they receive emotional support both during and after the patient has passed away.
  • Staff is available 24 hours a day, 7 days a week to assist you.
  • Services are provided in your home, assisted living, skilled nursing facility or hospital.
  • Staff are specially trained in pain and symptom management.
  • Prescription drugs, over the counter medications and medical equipment and supplies are available to treat your life-limiting illness.

Levels of Care Provided

  • Routine Home Care: Regular hospice care provided in patient’s home.
  • Continuous Home Care: Provided during times of crisis in which a patient requires continuous nursing care to achieve palliation or management of acute medical symptoms. This intensive care is provided in the patient’s home or facility where they live.
  • General Inpatient Care: For pain control and symptom management that cannot effectively be provided in other settings.
  • Respite Care: Short-term inpatient care provided to the patient when necessary for the purpose of providing a break in caregiving to the patient’s caregiver(s). It is only provided on an occasional basis, for a maximum of five days approximately every 90 days.
